GENEALOGY INFORMATION FORM

We welcome the descendents of the MacNaughton & MacMillan Clans to share their ancestry with our Clan Genealogist, Janet MacNaughton Miller to be included in our records. This is done by completing this FORM.

Due to security issues w/ Javascript, the computer industry has started to phase out its usage of Javascript. What this means to us it that several of the buttons I originally added for "Ease of Use” no longer work. But, at the top of the form (assuming you use Adobe Acrobat Reader), there are PRINT & SAVE buttons.

Form Input Instructions

There are 3 ways to submit your Info  FORM:

• Fill out the form online, Save File & email it directly to the This email address is being protected from spambots. You need JavaScript enabled to view it. (recommended).  FORM 1
• Fill out the form online, print it out, then snail-mail to the Genealogist.  FORM 2
• Print the form, fill it out by hand & mail it to the Genealogist (please contact us by This email address is being protected from spambots. You need JavaScript enabled to view it. to request the mailing address). FORM 3

To email a completed form, just save the filled-out form to your computer then attach the file (named “Genealogy Form”) to an email addressed to the This email address is being protected from spambots. You need JavaScript enabled to view it..

Explanations:

• The “Member” is the person whose family is part of the MacNaughton family line.
• Maiden Name: This name is very important for running research searches, PLEASE enter it if possible.
• Date Fields: All date fields must be entered as DDMONYYYYY, just numbers & letters, no special characters (slashes, hyphens, dots). Ex. 01JAN1800. If any part of the date is unknown, please just leave the field blank & make a note in the AIF box at the bottom of the form w/ the known info.
• Place Fields: City, State & Country (if outside the US) are needed but additional info like Church, Hospital, Cemetery names are helpful.
• Additional Information Field (AIF): This box is at the end of the form. This is for entering additional info such as Divorces, Additional Marriages, Incomplete Dates, Etc. Please use the same format when entering this info. The Generation # is EXTREMELY important & must be entered with the info below. This tells us where to enter your information into the genealogy database.

For an Additional Marriage

For a Divorce

• Gen. #, Name
